What To Expect?

Age Spots | Solar Lentigo| Sun Spots

Age spots are small, flat dark areas on the skin. They vary in size and usually appear on areas exposed to the sun, such as the face, hands, shoulders and arms. Age spots are also called sunspots, liver spots and solar lentigines.

Age spots can look like cancerous growths. True age spots don't need treatment, but they are a sign the skin has received a lot of sun exposure and are an attempt by your skin to protect itself from more sun damage. You can help prevent age spots by regularly using sunscreen and avoiding the sun.

Symptoms

Age spots may affect people of all skin types, but they're more common in adults with light skin. Unlike freckles, which are common in children and fade with no sun exposure, age spots don't fade.

Age spots:

  • Are flat, oval areas of increased pigmentation
  • Are usually tan to dark brown
  • Occur on skin that has had the most sun exposure over the years, such as the backs of hands, tops of feet, face, shoulders and upper back
  • Range from freckle size to about 1/2 inch (13 millimeters) across
  • Can group together, making them more noticeable

When To See a Doctor

Age spots don't require medical care. Have your doctor look at spots that are black or have changed in appearance. These changes can be signs of melanoma, a serious form of skin cancer.

It's best to have any new skin changes evaluated by a doctor, especially if a spot:

  • Is black
  • Is increasing in size
  • Has an irregular border
  • Has an unusual combination of colors
  • Is bleeding

Causes

Age spots are caused by overactive pigment cells. Ultraviolet (UV) light speeds up the production of melanin, a natural pigment that gives skin its color. On skin that has had years of sun exposure, age spots appear when melanin becomes clumped or is produced in high concentrations.

Use of commercial tanning lamps and beds also can cause age spots.

Prevention

To help avoid age spots and new spots after treatment, follow these tips for limiting your sun exposure:

  • Avoid the sun between 10 a.m. and 2 p.m. Because the sun's rays are most intense during this time, try to schedule outdoor activities for other times of the day.
  • Use sunscreen. Fifteen to 30 minutes before going outdoors, apply a broad-spectrum sunscreen with a sun protection factor (SPF) of at least 30. Apply sunscreen generously, and reapply every two hours — or more often if you're swimming or perspiring.
  • Cover up. For protection from the sun, wear tightly woven clothing that covers your arms and legs and a broad-brimmed hat, which provides more protection than does a baseball cap or golf visor.Consider wearing clothing designed to provide sun protection. Look for clothes labeled with an ultraviolet protection factor (UPF) of 40 to 50 to get the best protection.

Acne, Acne Scars and Acne Surgery

Acne is a skin condition which has plugged pores (blackheads and whiteheads), inflamed pimples (pustules), and deeper lumps (nodules). Acne occurs on the face, as well as the neck, chest, back, shoulders, and upper arms. Although most teenagers get some form of acne, adults in their 20's, 30's, 40's, or even older, can develop acne. Often, acne clears up after several years, even without treatment. Acne can be disfiguring and upsetting to the patient. Untreated acne can leave permanent scars; these may be treated by your dermatologist in the future. To avoid acne scarring, treating acne is important.

Types of Acne and How Acne Forms

Acne is not caused by dirt. Testosterone, a hormone which is present in both males and females, increases during adolescence (puberty). It stimulates the sebaceous glands of the skin to enlarge, produce oil, and plug the pores. Whiteheads (closed comedones), blackheads (open comedones), and pimples (pustules) are present in teenage acne.

Early acne occurs before the first period and is called prepubertal acne. When acne is severe and forms deep "pus-filled" lumps, it is called cystic acne. This may be more common in males. Adult acne develops later in life and may be related to hormones, childbirth, menopause, or stopping the pill. Adult women may be treated at the period and at ovulation when acne is especially severe, or throughout the entire cycle. Adult acne is not rosacea, a disease in which blackheads and whiteheads do not occur.

Cleansing

Acne has nothing to do with not washing your face. However, it is best to wash your face with a mild cleanser and warm water daily. Washing too often or too vigorously may actually make your acne worse.

Diet

Acne is not caused by foods. However, if certain foods seem to make your acne worse, try to avoid them.

Cosmetics

Wear as little cosmetics as possible. Oil-free, water-based moisturizers and make-up should be used. Choose products that are "non-comedogenic" (should not cause whiteheads or blackheads) or "non-acnegenic" (should not cause acne). Remove your cosmetics every night with mild soap or gentle cleanser and water.

A flesh-tinted acne lotion containing acne medications can safely hide blemishes. Loose powder in combination with an oil-free foundation is also good for cover-up.
Shield your face when applying sprays and gels on your hair.

Treatments

Control of acne is an ongoing process. All acne treatments work by preventing new acne breakouts. Existing blemishes must heal on their own, and therefore, improvement takes time. If your acne has not improved within two to three months, your treatment may need to be changed. The treatment your dermatologist recommends will vary according to the type of acne.

Occasionally, an acne-like rash can be due to another cause such as make-up or lotions, or from oral medication. It is important to help your dermatologist by providing an updated history of what you are using on your skin or taking internally.
Many non-prescription acne lotions and creams help mild cases of acne. However, many will also make your skin dry. Follow instructions carefully.

Topicals

  • Your dermatologist may prescribe topical creams, gels, or lotions with vitamin A acid-like drugs, benzoyl peroxide, or antibiotics to help unblock the pores and reduce bacteria. These products may cause some drying and peeling. Your dermatologist will advise you about correct usage and how to handle side effects.
  • Before starting any medication, even topical medications, inform your doctor if you are pregnant or nursing, or if you are trying to get pregnant.

Special Treatments

  • Acne surgery may be used by your dermatologist to remove blackheads and whiteheads. Do not pick, scratch, pop, or squeeze pimples yourself. When the pimples are squeezed, more redness, swelling, inflammation, and scarring may result.
  • Microdermabrasion may be used to remove the upper layers of the skin improving irregularities in the surface, contour, and generating new skin.
  • Light chemical peels with salicylic acid or glycolic acid help to unblock the pores, open the blackheads and whiteheads, and stimulate new skin growth.
  • Injections of corticosteroids may be used for treating large red bumps (nodules). This may help them go away quickly.

Recovery After V-Beam Laser Treatment

You should avoid saunas, hot tubs, and other forms of excessive heat for 48 hours after treatment.  For the first two weeks avoid skincare products that may clog your pores or cause skin irritation. As with all skin treatments, it is critical to use a SPF 30 or higher sunscreen every time you go outdoors.

What is a chemical peel?

Types of Chemical Peels

Chemical peels are classified as superficial, medium, and deep peels.

Superficial Peel

This peel is designed to help smoothen coarse, dry skin, improve the texture of sun-damaged skin, and even out skin tone. The recovery is generally rapid and usually involves some minor flaking. A series of treatments is ordinarily recommended to obtain the desired result over a period of time. Typically, there is no down time and with appropriate sun protection, you can resume regular activities immediately.

Medium Peel

Medium peels contain a stronger potency of acids. Medium peels target the epidermis and upper dermis and cause the skin to slough within 5 - 7 days. Immediately after treatment the skin has a red appearance and is occasionally accompanied by swelling. Within a day the skin turns brown in color and after 3 or 4 days the skin initiates the peeling process.

Deep Peels

Deep peels produce the most dramatic results. This peel is recommended for treating wrinkles, scars, blemishes and, in some cases, precancerous skin lesions. A deep peel targets the dermis. In some cases, anesthesia is required during the procedure to reduce the warm to hot stinging sensation. After a deep peel, the skin is red and can result in some swelling and blistering. A crust may form and the skin will turn brown just prior to peeling. Many layers of the skin will peel over a period of one to two weeks. An ointment may be given to keep the area moist. Temporarily, the new skin will have the color and sensation of significant sunburn, which will gradually fade to your normal skin color.

Skin cancer is the most prevalent cancer in the United States. More than 3 million Americans are diagnosed with non-melanoma skin cancer(s), such as basal cell & squamous cell carcinoma each year. These two types of cancers are collectively known as keratinocyte carcinoma or (KC).

In individuals with a history of a prior KC, the risk of developing subsequent KC remains a significant concern. 

In our study using non-ablative fractional laser therapy, including 4 sessions spaced at 2-4 week intervals, w/ follow-up assessments, 1-3 months after the final treatment session 60% had greater reduction of the development of KC and had improved overall health of sun-damaged skin. There was also a lower recurrence rate of MOHS. Lowering the rate of infection directly related to lack of having an open wound.

Non-ablative fractional laser therapy has already demonstrated remarkable success with minimal downtime & discomfort as a gentle yet effective treatment of actinic keratosis (AK), the most common type of pre-cancerous skin conditions. What to expect during a Fraxel Laser "Regenerative" Re-Store/ Resurfacing Treatment?

Most patients describe an increased sensation of heat during the treatment. Prior to the procedure we will apply a topical anesthetic cream, which helps minimize any discomfort during treatment.

How many treatments will I need?

The number of treatments needed depends on your goals and constraints. Typically, patients require a minimum of 2 treatments @ 4 week intervals.

What skin types and what parts of the body can be treated?

Fraxel lasers can treat women and men of all ages and skin colors. Any area of the body can be treated: face, arms, legs, back. abdomen and buttocks. Fraxel treatments also work on delicate skin areas like the neck, chest and hands.

What kind of results can I expect?

Your skin will feel softer, look brighter and improve skin tone. Other benefits of treatment include: reducing the appearance of acne scaring, treatment of actinic keratosis, reduction of fine lines and wrinkles, smoother, fresher, younger looking skin, improved tone and texture, erase unwanted brown spots, age spots & melasma.

When do the results appear, and how long do they last?

There are both immediate and progressive results. Soon after the treatment, the surface of your skin will feel softer, look brighter and show improved tone & texture. The next 3 to 6 months will bring additional improvement as the deeper layers of the skin continue to heal. Results can last for several years depending on your age and skin condition.

What will I look like immediately after the procedure? Can I go right back to work?

You will experience redness and/or swelling resembling a bad sunburn after the procedure that diminishes over the next 3-4 days. The evening after your treatment, your face will feel hot, applying cool compresses will provide comfort. We recommend using the prescribed ointments after treatment during the healing process. Usually by days 4-5 you will begin to peel, revealing smoother, younger looking skin. Total downtime is usually 5-7 days. Dr. Robinson will work with you to determine the best option within your lifestyle and work constraints.

Post Fraxel Treatment

HydraFacial MD is a godsend one week post Fraxel to speed up the shedding process that re-surfacing procedures inevitably induce while deeply hydrating the skin. Once healing is done, it is important to stay out of the sun as much as possible in order to maintain the results. The use of sunscreens, Retinol and skin brightening products will help keep hyperpigmentation at bay.

Can I get a Fraxel treatment if I already had other procedures?

Fraxel Laser compliments other cosmetic procedures, such as dermal fillers, Botox, TempSure Envi & HydraFacial MD.

Acne Treatments

Control of acne is an ongoing process given that acne can be a chronic problem. All acne treatments work to reduce current acne pimples but work better at preventing new acne breakouts. Acne treatments usually take 6 to 12 weeks to become fully effective. During that time existing blemishes may take time to heal and you may experience temporary worsening before improvement. Chemical peels and laser treatments can speed the time to improvement and often help clear pigmentation and scars. . Your treatment will vary according to the type of acne i.e. acne cysts, pustules, whiteheads or blackheads.

Occasionally, an acne-like rash can be due to another cause such as make-up or lotions, or from oral medication. It is important to help Dr. Robinson by providing an updated history of what you are using on your skin or taking internally. Many non-prescription acne lotions and creams help mild cases of acne. However, many will also make your skin dry. Follow instructions carefully.

Topicals

  • Dr. Robinson may prescribe topical creams, gels, lotions with vitamin A acid-like drugs, benzoyl peroxide, or antibiotics to help unblock the pores and reduce bacteria. These products may cause some drying and peeling. They will advise you about correct usage and how to handle side effects.
  • Before starting any medication, even topical medications, inform your doctor if you are pregnant or nursing, or if you are trying to get pregnant.

Special Treatments

  • Acne surgery may be used by Dr. Robinson to remove blackheads and whiteheads. Do not pick, scratch, pop, or squeeze pimples yourself. When the pimples are squeezed, more redness, swelling, inflammation, pigmentation and scarring may result.
  • Lasers and microdermabrasion may be used to remove the upper layers of the skin improving irregularities in the surface, contour, and generating new skin.
  • Light chemical peels with salicylic acid or glycolic acid help to unblock the pores, open the blackheads and whiteheads, stimulate new skin growth and improve pigmentation (dark spots).
  • Injections of corticosteroids may be used for treating large red bumps (acne cysts). This may help them go away quickly and prevent scarring.

Oral

  • Antibiotics taken by mouth such as tetracycline, doxycycline, minocycline, or erythromycin are often prescribed for acne cyst or painful acne pimples.

Birth Control Pills

  • Birth control pills may significantly improve acne, and may be used specifically for the treatment of acne. It is also important to know that oral antibiotics may decrease the effectiveness of birth control pills. This is uncommon, but possible, especially if you notice break-through bleeding. As a precautionary measure use a second form of birth control.

Other Treatments

  • In cases of unresponsive or severe acne, Accutane (isotretinoin) may be used. Patients using isotretinoin must understand the side effects of this drug. Monitoring with frequent follow-up visits is necessary. Pregnancy must be prevented while taking the medication, since the drug causes birth defects.
  • Women may also use female hormones or medications that decrease the effects of male hormones to help their acne.
  • Laser, microdermabrasion and chemical peels can be helpful in treating acne as well.

Dr. Robinson will evaluate your ance and suggest the best treatment regimes considering your age, sex, and the type of acne you have.

Treatment of Acne Scarring

Dr. Robinson can treat acne scars by a variety of methods. Skin resurfacing with laser, dermabrasion, chemical peels, or electrosurgery can flatten depressed scars. Soft tissue elevation with subcision or filling agents such as Radiesse, Restyene, Juvederm, Cosmoderm or Cosmoplast can elevate scars. Scar revision with microexcision and punch grafting can correct pitted scars. Combinations of these surgical treatments can make a noticeable difference in appearance.
