Laser Skin Resurfacing

How does laser skin resurfacing work?

Laser energy can be used to target both damaged skin at the surface, as well as aging collagen in the deeper layers of skin. The effect of this treatment is two-fold: first, you’ll see an immediate improvement in skin texture at the superficial level. As the skin heals, the treated area will be resurfaced with the growth of new, fresh skin cells, resulting in smoother, healthier-looking skin. Second, the laser energy also works to improve skin beneath the surface by stimulating the body’s natural healing mechanisms and encouraging the production of new collagen and elastin in the coming months.* Found in abundance in young, healthy skin, collagen and elastin provide natural skin firmness and elasticity.

Laser skin resurfacing can improve skin texture and tone, reduce wrinkles and acne scarring, and promote healthier-looking, more vibrant skin.This treatment is excellent for acne scarring as well as sun damaged or aged wrinkled skin. Erbium Laser Resurfacing

Laser treatments can be an ideal alternative for treating hyperpigmentation in addition to sun damage and fine lines. There are quite a few laser treatments on the market, but we're here to share information about Erbium laser resurfacing, a treatment that does it all and treats everything from wrinkles, acne scars to sun damage and brown spots/age spots.  While this treatment provides a more youthful appearance it usually requires a week of down time.

How does the Erbium laser resurfacing work?

Erbium laser resurfacing uses light energy and is designed to remove the upper most superficial layers. Removing brown spots and benign and precancerous bumps/growths. The Erbium laser results in a narrower area of heating of adjacent tissues compared with other laser skin-resurfacing methods. This results in a more focused removal of old skin cell layers. And this means that a benefit of Erbium laser resurfacing is minimal burning of surrounding tissue and fewer side effects such as swelling, bruising, and redness, so the recovery time should be faster than with CO2 laser resurfacing. During the healing process new collagen is generated for a more youthful appearance.

Does it hurt?

With the use of a potent topical anesthetic the Erbium laser treatment is well tolerated.  Usually up to the first hour after treatment it feels like a bad sunburn.  After which it may feel sore and have some bruising and mild swelling for a few days.  Followed by a few days of skin flaking for another two to three days. Usually, the total recovery/healing on average takes about 10-14 days, depending on the problem and area that was treated.

Why the downtime and what are the benefits?

Despite the downtime, the benefits are that you require fewer laser treatments to obtain your goal of smoother, younger looking skin.

Laser Hair Removal

What should I know about laser hair removal?

Depending on the amount of hair being treated, the procedure varies from patient to patient. Generally, the treatment will include:

  • A test area will be done prior to the entire treatment. Your full treatment may be done on the same day or up to two weeks later, depending on the skin reaction.
  • Both you and your practitioner will wear safety eyewear to protect your eyes from the laser light during the procedure.   
  • A small hand piece will be used that touches the skin to deliver the laser light. Cryogen spray will cool the skin just before each laser pulse.
  • Most patients experience little discomfort at the treatment site because of the Dynamic Cooling Device.
  • The degree of discomfort varies with each person, depending on his/her pain threshold. Topical anesthesia may also be requested based on individual preferences and tolerances. Be sure to discuss this with your practitioner in advance of the laser treatment.
  • The procedure can take as little as 10 minutes or up to an hour, depending on the area being treated.
  • The treated area may become pink or red within 30 minutes of treatment; this may last from a few hours to several days and is relieved with the application of a cooling compress, cold gel pack or cooling gels.

How many treatments will I need?

Hair grows in cycles. The number of treatments required depends upon your skin color, hair color and coarseness of hair and location of treatment. Everyone will require at least 2-3 treatments, as the process is only effective on hair during the early growing cycle. Repeat sessions will be necessary to treat these follicles when they re-enter the early growth phase.

How does the laser treatment work?

The laser creates a beam of high-intensity light that penetrates deeply into skin tissue, where it delivers a controlled amount of therapeutic heat to the targeted hair or spider veins. Together, the long-pulse laser and DCD cooling offer optimal treatment with minimal side effects.

What can I expect during treatment?

Treatment will take place in Dr. Bruce Robinson's New York office. You will wear safety eyewear to protect your eyes from the laser light. Dr. Bruce Robinson will guide the laser to the treatment areas with a small handpiece. Each laser pulse may be preceded and followed by a short pulse of cooling spray. A treatment could take as little as a few minutes to as long as an hour, depending on the area being treated.

Does it hurt?

Some patients may feel a slight discomfort with each laser pulse, sometimes described as the snapping of a rubber band on the skin. This discomfort resolves in several seconds and is minimized by Candela's DCD cooling. For some patients, a topical anesthetic may be used prior to treatment. Be sure to discuss this with Dr. Bruce Robinson before the procedure if applicable.

How many treatment sessions will I need?

The number of sessions depends on the treatment you require and the condition of your skin. Multiple treatments may be required, but many factors impact the efficacy of individual laser treatments, including the severity of the condition being treated.

How can I prepare for treatment?

Your skin should be thoroughly cleaned and dried, removing any makeup, creams, oils, topical anesthetics or self-tanning products before laser treatment. If you have had a history of herpes simplex virus and are scheduled for facial vain or hair removal treatment, prophylactic antiviral therapy may be prescribed. The intended treatment area should not be exposed to sunlight or tanning booth light for 14 days prior to treatment.

What can I exepct after treatment?

A cool compress or gel pack may be applied for comfort following your treatment. If any redness or swelling should occur it resolves within a few hours.

What precautions should I take after treatments?

After laser treatment, prevent sun exposure by applying sunblock (SPF 30 or above). In the first few days after treatment, don't scrub or rinse the treated areas with abrasive skin cleansers. Dr. Bruce Robinson may advise additional precautions.

Are there side effects?

Your risks are minimal. Although rare, effects may include redness, swelling, hyperpigmentation, hypopigmentation, blistering and purpura (a laser bruise) in the treated area--these usually do not last more than a few hours to a few days. Permanent side effects are rare. Be sure to discuss potential side effects with Dr. Bruce Robinson.

Is this procedure covered by insurance?

Skin cancer is the most prevalent cancer in the United States. More than 3 million Americans are diagnosed with non-melanoma skin cancer(s), such as basal cell & squamous cell carcinoma each year. These two types of cancers are collectively known as keratinocyte carcinoma or (KC).

In individuals with a history of a prior KC, the risk of developing subsequent KC remains a significant concern. 

In our study using non-ablative fractional laser therapy, including 4 sessions spaced at 2-4 week intervals, w/ follow-up assessments, 1-3 months after the final treatment session 60% had greater reduction of the development of KC and had improved overall health of sun-damaged skin. There was also a lower recurrence rate of MOHS. Lowering the rate of infection directly related to lack of having an open wound.

Non-ablative fractional laser therapy has already demonstrated remarkable success with minimal downtime & discomfort as a gentle yet effective treatment of actinic keratosis (AK), the most common type of pre-cancerous skin conditions. Acne Treatments

Control of acne is an ongoing process given that acne can be a chronic problem. All acne treatments work to reduce current acne pimples but work better at preventing new acne breakouts. Acne treatments usually take 6 to 12 weeks to become fully effective. During that time existing blemishes may take time to heal and you may experience temporary worsening before improvement. Chemical peels and laser treatments can speed the time to improvement and often help clear pigmentation and scars. . Your treatment will vary according to the type of acne i.e. acne cysts, pustules, whiteheads or blackheads.

Occasionally, an acne-like rash can be due to another cause such as make-up or lotions, or from oral medication. It is important to help Dr. Robinson by providing an updated history of what you are using on your skin or taking internally. Many non-prescription acne lotions and creams help mild cases of acne. However, many will also make your skin dry. Follow instructions carefully.

Topicals

  • Dr. Robinson may prescribe topical creams, gels, lotions with vitamin A acid-like drugs, benzoyl peroxide, or antibiotics to help unblock the pores and reduce bacteria. These products may cause some drying and peeling. They will advise you about correct usage and how to handle side effects.
  • Before starting any medication, even topical medications, inform your doctor if you are pregnant or nursing, or if you are trying to get pregnant.

Special Treatments

  • Acne surgery may be used by Dr. Robinson to remove blackheads and whiteheads. Do not pick, scratch, pop, or squeeze pimples yourself. When the pimples are squeezed, more redness, swelling, inflammation, pigmentation and scarring may result.
  • Lasers and microdermabrasion may be used to remove the upper layers of the skin improving irregularities in the surface, contour, and generating new skin.
  • Light chemical peels with salicylic acid or glycolic acid help to unblock the pores, open the blackheads and whiteheads, stimulate new skin growth and improve pigmentation (dark spots).
  • Injections of corticosteroids may be used for treating large red bumps (acne cysts). This may help them go away quickly and prevent scarring.

Oral

  • Antibiotics taken by mouth such as tetracycline, doxycycline, minocycline, or erythromycin are often prescribed for acne cyst or painful acne pimples.

Birth Control Pills

  • Birth control pills may significantly improve acne, and may be used specifically for the treatment of acne. It is also important to know that oral antibiotics may decrease the effectiveness of birth control pills. This is uncommon, but possible, especially if you notice break-through bleeding. As a precautionary measure use a second form of birth control.

Other Treatments

  • In cases of unresponsive or severe acne, Accutane (isotretinoin) may be used. Patients using isotretinoin must understand the side effects of this drug. Monitoring with frequent follow-up visits is necessary. Pregnancy must be prevented while taking the medication, since the drug causes birth defects.
  • Women may also use female hormones or medications that decrease the effects of male hormones to help their acne.
  • Laser, microdermabrasion and chemical peels can be helpful in treating acne as well.

Dr. Robinson will evaluate your ance and suggest the best treatment regimes considering your age, sex, and the type of acne you have.

Treatment of Acne Scarring

Dr. Robinson can treat acne scars by a variety of methods. Skin resurfacing with laser, dermabrasion, chemical peels, or electrosurgery can flatten depressed scars. Soft tissue elevation with subcision or filling agents such as Radiesse, Restyene, Juvederm, Cosmoderm or Cosmoplast can elevate scars. Scar revision with microexcision and punch grafting can correct pitted scars. Combinations of these surgical treatments can make a noticeable difference in appearance.
