When will I see results? How long do they last?

Are there any side effects?

The skin might appear flushed at first, but the redness should disappear within a few hours. What is a treatment like?

Treatment begins with a warm ultrasound gel applied to the selected skin area.  Radiofrequency energy is delivered via a wand which massages and gently heats the skin in relaxing small circular motions. Tissue resistance to RF energy at the dermal and subdermal junction causes an increase in temperature. This temperature is modulated by the TempSure system to safely maintain the skin at appropriate temperature levels. As the skin temperature increases during the procedure, new collagen is created. the new collagen fibers are tight, dense and neatly organized, giving you tighter, younger-looking skin. Once the procedure is complete the skin temperature drops back to normal. Cosmetic Surgical Excision of Cysts, Moles & Scars

A surgical excision is an in-office procedure in which a skin lesion is removed by a dermatologic surgeon. This technique involves the use of local numbing medicine which is injected into and around the target area and followed by sharp dissection of the lesion from surrounding normal tissue. The most common scenarios for utilization of this technique include: treatment of skin cancers, atypical or suspicious moles (nevi), and cysts.

Surgical Excision of Cysts

Cysts may remain asymptomatic, but if they continue to grow or become painful surgical excision is the treatment of choice. The cyst consists of a wall that makes the contents. To insure that the cyst does not return, it is best to completely excise it, including the wall, to prevent its recurrence.

Surgical Excision of Moles

Moles often become more raised over time. Occasionally they become irritated or are cut when shaving. A shave excision removes the raised portion of the mole. As a general rule, a complete excision is only performed if a mole is changing or when a complete excision will yield a better cosmetic result. Either of these treatments is performed under local anesthesia in our office. The shave excision takes less than a minute and a complete excision under a half hour. The functional and cosmetic results are excellent.

Surgical Excision of Scars

This option is typically for scars that are deeply pitted, raised, or for older surgical scars that did not heal properly. The scar tissue is removed, followed by careful stitching of the skin to leave a much smaller, thinner scar line or to change the scars shape and/or direction so it is less obvious.

Microdermabrasion

Microdermabrasion is a non-surgical, minimally invasive treatment used to renew overall skin tone and texture. It can improve the appearance of sun damage, wrinkles, fine lines, age spots, acne scarring, melasma, and other skin-related concerns and conditions. 

The procedure uses a special applicator with an abrasive surface to gently sand away the thick outer layer of the skin to rejuvenate it. Microdermabrasion exfoliating treatment is helpful for those unhappy with scars, acne scars, sun-damaged skin, pigmentation spots, stretch marks, fine lines, and rough skin. Treatment can be performed anywhere on the body. This treatment works for scarring especially when used in conjunction with other modalities.

Just about everyone has a few obvious brown spots on their skin. Age spots, freckles, and various birthmarks are just a few of the commonly known marks generally referred to as pigmented lesions. What causes these unwanted pigmented lesions?

Melanin is what gives our skin its color. The amount of melanin in your skin determines if you have a light or dark complexion. Pigmented lesions are dark in color simply because the melanin is abnormally concentrated in one area of the skin. High concentrations of melanin can be due to various factors. Some types are present at birth, but most occur with age, or as a result of overexposure to the sun's damaging ultraviolet rays.

How does the Medlite, Q-switched Nd:Yag Laser remove unwanted pigments in the skin?

The MedLite® laser is one of the most technologically advanced lasers for removing pigmented lesions. The laser removes these pigments with the energy of light. The specialized light produced by the MedLite laser is absorbed by the pigmented lesion. The unwanted pigment is then destroyed, thus removing or lightening the lesion.

What types of brown spots or pigmented lesions will the MedLite remove?

The MedLite Laser is most commonly used for removing brown and red spots, age spots, liver spots, sun spots, freckles, birthmarks, such as cafe-au-lait spots and Nevus of Ota, broken capillaries, tattoos and angiomas.

Does the MedLite laser remove my normal skin pigmentation also?

No, the laser light of the MedLite laser was specifically designed to effectively target the dark concentrations of melanin. After the abnormal amounts of melanin are removed, the skin's natural regenerative system will replace the normal melanin over time, returning the skin to its natural color.

Is the process painful?

The MedLite laser emits light in very short pulses. The impact of the energy from the power pulse of light is similar to the snap of a thin rubber band. After the treatment, the area will feel similar to a light sunburn. Typically any discomfort will disappear within a day or so. After the redness clears, the skin returns to its natural color, texture and tone.

How many treatment sessions will it take to remove the pigmented lesion?

Most common pigmented lesions are removed within one to two treatments. Some deeper lesions will require more treatments.

Can the excess pigmentation come back?

Common pigmented lesions do not return. Some birthmarks may return after a period of several months to a year. If that should occur, the procedure can be repeated.

What type of post-treatment care is necessary?

Usually, an antibacterial ointment will be applied to the area immediately after treatment. The treated area should be kept clean with a continued application of any ointment recommend to you by Dr. Robinson. The treated area should not be scrubbed.

For Tattoos

The MedLite laser - a good choice for tattoo removal.

Decorative tattoos have a history dating back at least 5000 years and the desire to remove them has probably existed for just as long. Early attempts to remove tattoos have had less than desirable results. The use of dermabrasion, saltabrasion and Argon or CO2 lasers have left behind scars in place of the tattoo. The advent of Q-Switched lasers has permitted the removal of most tattoo inks with a very low risk of scarring. The MedLite laser; Q-Switched Nd: YAG laser is the laser of choice for tattoo removal. The MedLite laser can significantly lighten or remove a wide variety of tattoo inks.

How does the MedLite laser remove the tattoo?

The MedLite laser removes tattoo ink with the energy of light. A laser is a device which is designed to produce one or more specific wavelengths of light. Tattoo ink is removed by using this specific wavelength of light which passes into the skin, but is absorbed by the ink. The rapid absorption of light energy causes the tattoo ink to break into tiny particles which can then be removed by the body's own natural filtering systems. The MedLite laser provides maximum tattoo removal while leaving the surrounding skin unharmed.

What type of tattoos can be removed?

The MedLite laser can remove professional, amateur (homemade), traumatic and surgical tattoos.

How many treatments will it take to remove a tattoo?

On the average, professional tattoos require 6-8 treatments, while amateur tattoos require 4-5 treatments, all spaced approximately 6-8 weeks apart. The number of treatments depends on the amount and type of ink used, and the depth of the ink in the skin.

Does the laser remove colored tattoos?

Dark (blue, black) and red inks will resolve the best. Oranges and purples usually fade as well. Green and yellow inks are the most difficult to remove, and additional treatments are needed to produce significant fading.

Will the tattoo completely disappear?

In over 95% of the cases fading of the tattoo will be accomplished. However, there are many types of tattoo inks in use today which are not regulated by the FDA. Not knowing which tattoo ink was originally used, or how deeply it was applied, makes it impossible for the physician to predict the degree of removal on any given tattoo.

Is the process painful?

The MedLite laser emits light in very short pulses. The impact of the energy from the pulse of light is similar to the snap of a small rubber band on the skin. The majority of patients do not require anesthesia, depending on the size and location of the tattoo. Anesthesia creams are available if needed.

What type of post-treatment care is necessary?

An antibacterial ointment and a dressing will be applied to the area immediately after treatment. Occasionally, there will be pinpoint bleeding associated with the treatment. The treated area should be kept clean. Follow office post laser treatment instructions for what to do during the days following the procedure. A shower can be taken the next day, although the treated area should never be scrubbed. If a scab forms it is vital that you do not pick it or scratch it.

Lyme Disease

You're more likely to get Lyme disease if you live or spend time in grassy and heavily wooded areas where ticks carrying Lyme disease thrive. It's important to take common-sense precautions in tick-infested areas.

Risk factors

Where you live or vacation can affect your chances of getting Lyme disease. So can your profession and the outdoor activities you enjoy. The most common risk factors for Lyme disease include:

  • Spending time in wooded or grassy areas. In the United States, deer ticks are found mostly in the heavily wooded areas of the Northeast and Midwest. Children who spend a lot of time outdoors in these regions are especially at risk. Adults with outdoor jobs also are at increased risk.
  • Having exposed skin. Ticks attach easily to bare flesh. If you're in an area where ticks are common, protect yourself and your children by wearing long sleeves and long pants. Don't allow your pets to wander in tall weeds and grasses.
  • Not removing ticks promptly or properly. Bacteria from a tick bite can enter your bloodstream if the tick stays attached to your skin for 36 to 48 hours or longer. If you remove a tick within two days, your risk of getting Lyme disease is low.

Early signs and symptoms

A small, red bump, similar to the bump of a mosquito bite, often appears at the site of a tick bite or tick removal and resolves over a few days. This normal occurrence doesn't indicate Lyme disease.

However, these signs and symptoms can occur within a month after you've been infected:

  • Rash. From three to 30 days after an infected tick bite, an expanding red area might appear that sometimes clears in the center, forming a bull's-eye pattern. The rash (erythema migrans) expands slowly over days and can spread to 12 inches (30 centimeters) across. It's typically not itchy or painful but might feel warm to the touch.
  • Erythema migrans is one of the hallmarks of Lyme disease, although not everyone with Lyme disease develops the rash. Some people develop this rash at more than one place on their bodies.
  • Other symptoms. Fever, chills, fatigue, body aches, headache, neck stiffness and swollen lymph nodes can accompany the rash.
  • Seek immediate treatment If the tick has been attached for more than 24 hours, or you see a red, bullseye spot emerge at the site or anywhere on the body or experience flu-like symptoms

Later signs and symptoms

If untreated, new signs and symptoms of Lyme infection might appear in the following weeks to months. These include:

  • Erythema migrans. The rash may appear on other areas of your body.
  • Joint pain. Bouts of severe joint pain and swelling are especially likely to affect your knees, but the pain can shift from one joint to another.
  • Neurological problems. Weeks, months or even years after infection, you might develop inflammation of the membranes surrounding your brain (meningitis), temporary paralysis of one side of your face (Bell's palsy), numbness or weakness in your limbs, and impaired muscle movement.

Less common signs and symptoms

Several weeks after infection, some people develop:

  • Heart problems, such as an irregular heartbeat
  • Eye inflammation
  • Liver inflammation (hepatitis)
  • Severe fatigue

When to see a doctor ...

If you've been bitten by a tick and have symptoms, only a minority of tick bites lead to Lyme disease. The longer the tick remains attached to your skin, the greater your risk of getting the disease. Lyme infection is unlikely if the tick is attached for less than 36 to 48 hours.

If you think you've been bitten and have signs and symptoms of Lyme disease — particularly if you live in an area where Lyme disease is common — contact your doctor. Treatment for Lyme disease is more effective if begun early. 

Visit your doctor even if signs and symptoms disappear — the absence of symptoms doesn't mean the disease is gone. Untreated, Lyme disease can spread to other parts of your body for several months to years after infection, causing arthritis and nervous system problems. Ticks can also transmit other illnesses, such as babesiosis and Colorado tick fever.

Prevention

The best way to prevent Lyme disease is to avoid areas where deer ticks live, especially wooded, bushy areas with long grass. You can decrease your risk of getting Lyme disease with some simple precautions:

  • Cover up. When in wooded or grassy areas, wear shoes, long pants tucked into your socks, a long-sleeved shirt, fitted at the wrists, a hat, tuck in hair if possible and gloves.
  • Wear white or light-colored clothing to make it easier to see ticks.
  • Try to stick to trails and avoid walking through low bushes and long grass. Keep your dog on a leash.
  • Wear shoes, no bare feet or sandals.
  • Use insect repellents. Apply insect repellent with a 20% or higher concentration of DEET to your skin. We recommend Deep Woods Off. Parents should apply repellant to their children, avoiding their hands, eyes and mouth.
  • Keep in mind that chemical repellents can be toxic, so follow directions carefully. Apply products with permethrin to clothing or buy pretreated clothing.
  • Do your best to tick-proof your yard. Clear brush and leaves where ticks live. Mow your lawn regularly. Stack wood neatly in dry, sunny areas to discourage rodents that carry ticks.
  • Check your clothing, yourself, your children and your pets for ticks. Be especially vigilant after spending time in wooded or grassy areas. Deer ticks are often no bigger than the head of a pin, so you might not discover them unless you search carefully.
  • It's helpful to shower as soon as you come indoors. Ticks often remain on your skin for hours before attaching themselves. Showering and using a washcloth might remove unattached ticks.
  • Don't assume you're immune. You can get Lyme disease more than once.
  • Remove a tick as soon as possible with tweezers. Gently grasp the tick near its head or mouth. As close to the skin as possible. Don't squeeze or crush the tick, but pull carefully and steadily. Once you've removed the entire tick, dispose of it by placing it in a container marked with the name of its host, area location, and the place of attachment on the body. 
  • Then call your physician to determine if treatment is warranted. Most doctors will send the suspect on to a state lab for analysis at no charge

Laser skin tightening is a nonsurgical, minimally non-invasive cosmetic procedure. It may help reduce signs of aging on the skin. As skin ages, it tends to lose its elasticity and become less tight.

The body areas people most often target using laser skin tightening procedures include the:

  • face
  • jawline
  • neck
  • stomach

Skin aging is a natural process that occurs as people age. However, it could be accelerated by such factors as sunlight, stress, and chemicals. Skin aging is affected by numerous genetic and environmental factors that can appear as wrinkles, abnormal pigmentation, skin weakness, and telangiectasia.

Laser Features and Skin Aging Treatment

Different types of lasers for skin rejuvenation are ablative lasers, non-ablative lasers, and fractional lasers

Ablative Lasers

These kinds of lasers have been used to treat scars, pigmentations, and rhytides by removing the epidermis and heating dermis. However, ablative lasers reduce time of treatment and cause a more difficult recovery process, these lasers create the most dramatic impressive results. For severe facial wrinkles, pigmentation, and skin challenges, ablative lasers are often the preferred treatment. The most common ablative lasers used for skin rejuvenation is the Nd:YAG.

Non-ablative Lasers

Non-ablative laser resurfacing demonstrates one of the main developments in procedural dermatology over the past decade and has become the treatment of selection for a broad range of aesthetic indications. However, safety concerns related to their use in darker skin types have remained. These lasers are less destructive than ablative lasers and tighten the skin by naturally stimulating collagen production in the dermis; the epidermis is protected through skin cooling. This type of laser is less aggressive than the Ablative laser options.

Fractional Lasers

What can I expect during the treatment?

Laser skin tightening is performed in the office. You will wear safety eyewear to protect your eyes.  Each laser pulse may be preceded and followed by a short puff of cooling spray.  A treatment can take as little as a few minutes to as long as an hour, depending on the area and condition being treated.  

Does it hurt?  

Patients will have a topical numbing cream applied approximately 30 minutes before their treatment.  Some patients may feel a slight discomfort with each laser pulse, sometimes described as the snapping of a rubber band on the skin.  Should this discomfort occur, it resolves in several seconds.  

How many treatment sessions will I need?

Usually 3 to 5 treatments at monthly intervals.  

How do I prepare for the treatment?

If you have a history of cold sores, prophylactic treatment with antiviral therapy (Valtrex, Famvir, or Acyclovir pills) may be started before your laser treatment.  The intended treatment area should not be exposed to sunlight or tanning booth light for 14 days prior to treatment.

What can I expect after the treatment?  

Your face will feel hot, applying cool compresses will provide comfort. Any redness or swelling should resolve within a few days. During the healing process your skin will originally appear "sunburned" and have a sandpaper feel and then peel 4-7 days after your treatment. By day 7 you will begin to reveal a softer, brighter and improved skin tone.  Treating yourself to a HydraFacial MD one- two weeks post Fraxel will help expedite the shedding of old skin and infuse intense hydration. Other benefits of Fraxel laser treatment include: reducing the appearance of acne scaring, treatment of actinic keratosis, reduction of fine lines and wrinkles, smoother, fresher, younger looking skin, improved tone and texture, erase unwanted brown spots, age spots & melasma.

What precautions should I take after the treatment?

After laser treatment, prevent sun exposure and apply sunblock (SPF 30 or greater).  Apply generous amounts of moisturizer often to keep skin hydrated. Avoid scrubs or abrasive skin cleansers the first few days after your treatment.

Are there side effects?  

Your risks are minimal.  Side effects may include redness, swelling, hyperpigmentation, hypopigmentation, blistering and a bruise in the treated area--if they do occur, it does not usually last more than a week.  Permanent side effects are rare.

Intralesional Steroid Injections

What is intralesional steroid injection used for?

Intralesional Steroid Injections, or Intralesional Kenalog is a common treatment used in dermatology & are a standard treatment for many skin diseases including: acne lesions, cysts, alopecia (bald spots) hair loss, eczema, abundant scar tissue (keloids, hypertrophic scars), psoriasis, lichen simplex chronicus, skin lupus, granuloma annulare, and more. Intralesional injections are extremely helpful, as they are able to bypass the outer defensive layers of the skin to reach an area that will make the greatest impact on your condition. It is common for a patient to need multiple injections, as different cases vary in severity. The goal of these injections is ideal for quick healing to reduce the inflammation within the skin, and relive pain and discomfort as well as improving the appearance. This treatment is usually reserved for tender, swollen acne nodules, or cysts that aren't responding to typical acne therapies. Prompt treatment with intralesional injections can often prevent scarring.

What are the side effects of Intralesional Steroid Injections?

  • This injection is a form of steroid, which some may have an allergic reaction to. It is not very likely that you will be allergic, but it is possible may happen after injection.
  • It is possible that the skin may become thinner in the area of injection (atrophy). This is not very likely, but could result in a dimpling of the skin. This tends to recover after some time.
  • Bleeding may occur after any type of injection including intralesional steroids. It is also possible that the blood vessels surrounding the injection site may become more prominent after injection.
  • Pigmentation can develop around the injection site.
  • As with most injections, there is a chance that pain will be felt. Each area of the body has a different level of sensitivity for each individual, so discomfort is possible. Local anesthetics can be used if necessary to help with pressure or pain during the procedure.
  • There is a slight possibility that a treatment may be ineffective, or that the condition may return after the injection.
